- Income Statement issued by the employer
- IC/IB (original)
- Photocopy of the first 3 and last 2 pages of the workbook stamped by employer and certified "according to the original"
- Statement regarding the monthly financial obligations of the customer and his/her family and the litigations with the third parties (original, standard form issued by the bank)
